The Community Rehabilitation Residence (CRR) Level I program provides rehabilitative services to male adults with chronic and persistent mental illness. The four-bed, group home is located in
The individual in this role also supports and works with the Intellectual & Developmental Disabilities (IDD) Crisis Services team. The focus of this program is to provide time-limited home and community based crisis stabilization services that support individuals with developmental disabilities in a residential setting. This home is also located in Greenville, GA next door to the CRR Level III home. An essential component of this program is the assessment of the individual situation to determine the appropriate response to the crisis. Crisis services occur through intensive on-site or off-site supports.
The Social Service Technician provides direct care to the residents in both programs as scheduled/needed. Responsibilities include but are not be limited to the following:
□ Provides appropriate input to the treatment team regarding client’s physical/psychosocial status based on interaction with and observation of consumer’s behavior
□ On a daily basis, accurately documents each client’s progress in the company's electronic health record (EHR) system based on observed behaviors and interactions
□ Assists in planning for and facilitates structured group activities within the home as well as in the community
□ Supervises self-administration of medications
□ Assist client(s) with daily living skills such as laundry, cleaning, cooking, and ambulatory task(s).
□ Transports, or makes arrangements to transport, client’s to appointments within organization or to community support services in a timely manner
□ Notifies appropriate team members) of any unusual client incident/situation in a timely manner
□ Prepares meals as needed and ensures the home is cleaned daily for health and safety purposes
□ Effectively manages behavior in a safe and non-confrontational manner
□ Provides appropriate emotional support and encouragement to clients and families and assists in the resolution of minor problems
□ Provides appropriate information and/or input in meetings pertaining to case management and/or case collaboration
The individual in this role must have a clean driving record. A pre-employment drug test and criminal background check is required upon hire.
