Under broad supervision, provides a broad range of social services to patients/clients/consumers and their families. Conducts patient/client/consumer intakes, participates in treatment team planning, and performs crisis intervention. Assists in dealing with personal and social problems. May provide supportive counseling to consumers and families and/or serve as a liaison for social services. May perform case management duties. Performs lead worker functions. Performs job responsibilities with minimal supervision. Serves as job expert or organization resource in assigned areas. Performs lead worker functions including assigning and reviewing subordinates work. Assists clients in locating and utilizing community resources including legal, medical, financial assistance, and other referral services. Completes patient/client/consumer intakes based on interviews with patients/clients/consumers, their families, significant others and appropriate community agencies. Counsels clients living in group homes and halfway houses; assists in pre-release and release planning. Implements and organizes the delivery of specific social services within the community. Implements life skills workshops and programs in behavior management, youth services, community and social services. Maintains contact with other social service agencies and health care providers involved with clients to provide information and obtain feedback on client's overall progress. Maintains program statistics for purposes of evaluation and research. Monitors patient's/client's/consumer's progress towards treatment goals. Prepares intake reports and case history records. Provides crisis intervention and emergency shelter services. Provides short-term case management and referral services to clients with emergency situations

Summary/Objective: 

Under general supervision, performs comprehensive clerical and support duties. Duties include Interacting with individuals with disabilities for the purposes of intake into the VR program, gathering information, entering data, preparing correspondence, uploading obtained records, and providing accurate information for individuals with disabilities. 

Essential Functions: 

Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions. 

  1. Provides positive customer experience for individuals interested in applying for VR services. 
  1. Reviews case file information to determine if services are available for students using excel spreadsheets.  
  1. Maintaining a positive, empathetic, and professional attitude toward customers at all times. 
  1. Interviews individuals with disabilities to obtain information and provides referral and application services. Uploads and documents support documentation and crucial client information. 
  1. Prepares and sends correspondence according to vocational rehabilitation policies and procedures. 

              6.    Comply with the policies of the VR program in regards to case file requirements and established timelines for individuals.

        7.  Enters accurate participant data in the client information system and captures reporting data to support an open case.

        8.   Maintains communication with clients, providers, and team members.

        9.   Observes rules of confidentiality as stated by departmental guidelines in the dissemination and interpretation of test results and other                    case file  information.

Preferred Education and Experience: 

Preference will be given to applicants who, in addition to meeting the required education and experience, possess one or more of the following: 

·                 Associate degree in a social service or related field; or certificate in administrative or social service or a related field

·                 12 months experience working with people with disabilities  

·                 12 months experience working in a remote customer service call environment

·                 Certified Notary in the State of Georgia in good standing 

Supervisory Responsibility: 

None required for this position. 

Work Environment: 

This job operates in a professional office and work from home environment. This role routinely uses standard office equipment such as laptop computers and smartphones. 

Physical Demands: 

While performing the duties of this job, the employee is regularly required to talk or hear. The employee frequently is required to stand; walk; use hands to finger, handle or feel; and reach with hands and arms. The employee is occasionally required to sit; climb or balance; and stoop, kneel, crouch or crawl. The employee must occasionally lift and/or move up to 10 pounds and occasionally lift and/or move up to 25 pounds. 

Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception and ability to adjust focus.

Working Conditions: 

Incumbents working in this job perform duties in an office setting. This is a 40-hour workweek requirement. This position may involve managing a high-volume workload and demands a high level of accuracy while working with deadlines. Regular attendance is an important part of being successful in this position. Reasonable accommodation can be made available to qualified individuals upon request. 

Position Type and Expected Hours of Work: 

This is a full-time position. Days and hours of work are Monday through Friday, 8:30 a.m. to 5 p.m. This position occasionally requires long hours and weekend work. 

Travel: 

Travel is primarily local during the business day, although some out-of-area and overnight travel may be expected.

As a condition of employment with Georgia Vocational Rehabilitation Agency, you must successfully complete a mandatory federal and national background check, pre- employment testing, and additional checks required for your job.
