CRISIS CSU – CASE MANAGER

 

ACCOUNTABILITY

Clinical Case Manager

SHIFT

Tuesday - Saturday - 12:00 pm - 8:45 pm


RESPONSIBILITIES INCLUDE

  • Completes community needs assessments as assigned and documents them according to policy and procedure.
  • Performs duties from a recovery oriented, client centered, and trauma informed perspective.
  • Ensures Individual Treatment Plans are completed timely as outlined in policy and procedure.
  • Assist individuals with short term or long-term placement (shelter, group home, inpatient hospitalization, etc.).
  • Provides skills groups and documents participation and response timely and as required by policy and procedure.
  • Works in PES and CSU (as needed).
  • Progress notes regarding discharge planning must include progress or lack of progress towards discharge.
  • Participates in daily Treatment Team Meeting/daily Shift reports.
  • Essential Learning Courses as scheduled.
  • Enters correct reauthorizations as needed into computer system.
  • Meets with individual’s family members as needed for discharge planning and documents results of the meeting.
  • Completes all discharge planning documentation timely and as outlined in policy and procedure.
  • Ensures discharge appointments are made, including any medical
  • Assist individuals with ensuring WRAP plans & SNAPS.
  • Schedules transport when consumers are unable to obtain.

 SKILL, COMPETENCY AND EDUCATION AND/OR LICENSURE REQUIREMENTS

  • Must possess basic computer and typing skills.
  • Must be able to successfully complete CPR, CPI, First Aid training and Defensive Driving.
  • Must be able to do Vital Signs. Must be able to provide a current three-year (3) Motor Vehicle Report that will allow for use of agency vehicles.
  • Ability to deal effectively with conflict situations, manage aggressive behavior, and use non-violent crisis intervention and behavioral management techniques.

INTER-RELATIONSHIPS

  • Able to interface in a professional and therapeutic manner with individual clients and groups of clients.
  • Able to communicate effectively with co-workers, internal and external clients, and the public.

WORKING CONDITIONS

  • In crisis intervention facility.
  • May be required to work holidays, weekends, or evenings or extended hours as needed.
  • Must be able to complete Crisis Prevention Intervention (CPI) training course with passing score, and to demonstrate ability to implement this in work setting as needed.
  • Must be able to complete CPR and First Aid training successfully.

NEEDED ATTRIBUTES

  • Must be well organized, consumer focused with excellent interpersonal and client service skills.
  • Ability to spell correctly and write coherently for documentation in consumer record.

MINIMUM REQUIREMENTS 

  • Bachelor’s degree in helping field (i.e., Social Work, Psychology, Human Services, etc.) required.

REFERRED QUALIFICATIONS

  • At least one year’s experience working in mental health or substance use disorder field preferred.

Bachelor's degree in a related area from an accredited college or university and two (2) years of job-related supervisory experience in a human service delivery program; or related certification and one (1) year of job-related supervisory experience in a human service delivery program; or two (2) years of experience at the lower level Social Svcs Prgm Mgr Spv (SSP023) or equivalent position.
