We are currently seeking Case Manager/Social Worker to join our team in making a difference in the lives of our patients every day.  We are located in the heart of Middle Georgia State where we enjoy abundant recreational opportunities, and a family friendly environment with good schools and opportunities for the entire family to be involved.

 

Summary of Duties

The Case Manager/Social Worker coordinates the overall plan of care for client with mild to moderate emotional and behavioral problems. Oversees patient evaluation and treatment planning, and monitors client progress, adjusting treatment plans as necessary. Acts as a liaison between patient/family and service delivery personnel to ensure clients are being effectively treated.

 

Responsibilities

·         Conducts patient/client/consumer intakes, participates in treatment team planning, and performs crisis intervention.

·         Assists in dealing with personal and social problems.

·         May provide supportive counseling to consumers and families and/or serve as a liaison for social services.

·         May perform case management duties.

·         Assists clients in locating and utilizing community resources including legal, medical, financial assistance, and other referral services.

·         Completes patient/client/consumer intakes based on interviews with patients/clients/consumers, their families, significant others and appropriate community agencies.

·         Counsels clients living in group homes and halfway houses; assists in pre-release and release planning. Implements and organizes the delivery of specific social services within the community.

·         Implements life skills workshops and programs in behavior management, youth services, community and social services.

·         Maintains contact with other social service agencies and health care providers involved with clients to provide information and obtain feedback on client's overall progress.

·         Maintains program statistics for purposes of evaluation and research.

·         Monitors patient's/client's/consumer's progress towards treatment goals.

·         Prepares intake reports and case history records.

·         Provides crisis intervention and emergency shelter services.

·         Provides short-term case management and referral services to clients with emergency situations

·         Will meet with clients a minimum of twice per month with at least 80% of contacts being face-to-face. 

·         Demonstrates clinical knowledge of mental health, addictive disease, and co-occurring disorder treatment.

·         Demonstrates documented proficiency (through education, training or experience) in client engagement, interviewing and recording. Focused on adults.

·         Conducts individual skills training using evidence-based interventions within the scope of service provided.

·         Assesses and documents response to interventions, modifies interventions as needed.

·         Demonstrates strong clinical judgment in illness management, relapse prevention, crisis intervention and development of recovery skills.

·         Develops therapeutic relationship as appropriate, with clients, family, significant others and recovery partners.

·         Serves as resource to staff in demonstration of up-to-date knowledge of OCSB, community, regional, and state resources.

·         Assists treatment team in problem solving regarding needed resources for clients.  

·         Insures timely communication with providers through proper exchange of written and verbal communication.

·         Documents referrals, discharges, resources or other issues for problem resolution and/or utilization as required in agency policy.

·         May serve as a liaison in community resource activities.

Minimum Qualifications

·         High school diploma or GED and two years’ experience in a social services related position or four years’ experience performing management (Business or Healthcare Management) responsibilities in a health care services environment or related field.

·         Two years of experience in Behavioral Health

·         Ability to travel a minimum of 70% of the time. (Local Travel)

 

Preferred Qualifications

·         Bachelor's degree in a social services related field and one year at the lower level

·         Six years’ experience performing management (Business or Healthcare Management) responsibilities in a health care services environment or related field.

·         Ability to travel a minimum of 70% of the time. (Local Travel)

 

Pre-employment Requirements:

  • Previous experience in mental/behavioral health field.
  • Possess a valid driver’s license
  • Submit to and pass a pre-employment:
    • Drug screen
    • DBHDD Background check
    • No more than four (4) points on a seven (7) year Motor Vehicle Report
      *If more than (4) points are on the MVR an approved DDS course to reduce points must be taken.

Knowledge & Skill

·         Must be a self-started and work independently;

·         Must be highly organized and efficient;

·         Computer skills required (internet, word processing and spreadsheet programs);

·         Must be able to perform assignments with minimal supervision;

·         Must be able to comprehend and perform oral and written instructions and procedures;

·         Must be capable of adapting to varying workloads and work assignments on a constant basis;

·         Must have effective comprehensive reading skills, strong communication skills, written and verbal.

High school graduate or equivalent *and* Six (6) months of related work experience.
