The incumbent will perform the following duties and responsibilities:
- Facilitate groups and/or one-on-one sessions in which you plan, coordinate, manage and implement Psycho-Educational lesson plans to help clients deal with difficulties and overcome dependencies
- Provide services to consumers concerning their Active Treatment; Include and not limited to classes, activities, and note writing
- Provide necessary coverage and support for peers and staff
- Participate in other special Events and assignments through the hospital
- Under general supervision, provide a broad range of social services to consumers
- Participate and collaborate on Treatment Teams
- Interview service users and assess their current condition, needs, strengths, and weaknesses
- Address each case as a unit and set tailored measurable goals
- Monitor and evaluate clients' progress and provide feedback to the team to modify treatment plans accordingly
- Offer information and feedback to the team on the best course of action during sessions
- maintain accurate and timely documentation and report on clients' status
- Act as a key-worker and cooperate with multidisciplinary teams
- Act as client advocate to resolve emergency problems in a crisis situation, and coordinate required services
- Collect information about client through interviews and observations
- Counsel clients in individual and group sessions to help them overcome dependencies, recover from illness, and adjust to life
- Evaluate client’s physical and mental condition based on review on observation and review of client’s information
- Maintain confidentiality of records
- Observes, monitor, evaluate, and record client’s behavior and progress according to goals described in treatment, care plan and group goals
- Submit required reports, documentation, and information in a timely manner
- Prepares and maintains records of client progress and services performed, reporting changes in client condition to manager or supervisor
This is temporary or hourly paid work performing professional social/counseling services. Positions in this job are established for less than 9 months or scheduled for less than 30 hours per week and are not eligible for benefits if the position status is temporary (i.e., SCOA 513). Positions in this job are established for greater than 9 months and scheduled for greater than 30 hours per week if the position status is permanent labor (i.e., SCOA 512). Normally permanent labor positions are eligible for Health Insurance benefits.
