The Georgia Department of Behavioral Health and Developmental Disabilities (DBHDD) is the state agency that focuses solely on policies, programs, and services for people with mental health challenges, substance use disorders, intellectual and developmental disabilities, or any combination of these. As Georgia’s public safety net, DBHDD operates six field offices and five hospitals throughout the state within three program areas: behavioral health, developmental disabilities and hospital services.
The main office for DBHDD is located in downtown Atlanta.

Job Title: Outcome Resolution Coordinator- Atlanta

Job Summary:
Under the limited supervision of the Support Coordination Manager, as part of the Office of Community Services, oversee the IDD Provider network with focus on recognize, refer and act process as part of support coordination work in service evaluation, recognition of unmet needs and impending risks, performance management and quality improvement.
Outcome Resolution Coordinator
• In coordination with DBHDD Central Office and Field Office staff, coordinate and provide oversight to ensure providers compliance with statewide policy, protocols, and standards
· Build rapport with DBHDD Provider agencies
• Aggregate state-wide Outcome Review report information to create a regional monthly and quarterly report.
• Oversee and provide technical assistance to providers’ development of Quality Improvement Plans and measures to improve the quality of ISP implementation, documentation, and service delivery
• Identify providers who are out of compliance with overdue referrals identified by Support Coordination, provide technical assistance to address unresolved concerns and oversee provider accountability for completing actions needed to promote quality outcomes for individuals served
• Work closely with the Division of Strategy, Technology and Performance to monitor quality indicators and draft changes to respond to changing programmatic needs to meet compliance
• Build and maintain relationships with support coordination agencies, field office staff and pertinent providers to promote collaboration and integration of work to ensure quality service delivery and accurate reporting
• Provide Onsight technical assistance and monitoring of IDD Providers critical incident reporting and monitoring of coaching and referral requests for timely action and completion based on coaching and referral reports
· Work with participant-direction family representatives for quality improvement for the delivery of self-directed services
Other Responsibilities
Ø Develop expertise in NOW/COMP waivers and service lines within Medicaid waivers
Ø Coordinate with DBHDD central office participant-direction staff to ensure support coordination understanding and completion of participant-direction related tasks to meet individuals’ needs in alignment waiver policy
Ø Give input based on field observation and trends for the drafting of policy and procedure manual updates related to waiver services, programmatic need changes per required timelines.
Ø Develop expertise in the IDD Case management system (IDD Connects, GAMMIS, IMAGE) for case reviews, open record requests, troubleshooting individual cases and statewide oversight of SC work
Ø Present recommendation of policy and procedure changes to the Support Coordination Manager with clear rationale and data to back the necessary change recommendations
Ø Work closely with Support Coordination Manager staff to facilitate programmatic outcomes and quality improvements
Ø Develop expertise in Person Centered Practices for service evaluation of Support Coordination documentation and Community Service Provider service delivery
Ø Develop expertise in the utilization of the Health Risk Screening Tool, the Support Intensity Scale, Communication Assessment Reports, clinical assessments, and other information to evaluate support needs for DD participants.
Ø Assess how providers support individuals in community inclusion and person-centered planning
Ø Provide updates for the drafting updates for DOJ deliverables of compliance Other duties as assigned
Ø Assist in the oversight of participant- direction families compliance with PD Policy and DBHDD program requirements.
Preferred Qualifications:
Bachelor’s degree with at least two years of program management experience. Degree in human service or government management related field from an accredited college or university and two years of related experience OR Master’s degree in a related field from an accredited college or university OR two years of experience at the lower level or position equivalent.
· Strong customer service and relationship building skills
· Analytical skills to include report writing and interpretation
· Excellent verbal and written communication skills
· Experience with intellectual and developmental disability services
