View Point Health is a community behavioral health center utilizing a team of psychiatrists, nurses, licensed counselors and social workers, and other professionals to provide services to individuals who need treatment and support to cope with mental illness, substance abuse, and intellectual and developmental disabilities. View Point Health serves uninsured, underinsured, low-income Medicaid, Medicare, war veterans and some private insurance across multiple locations with a full continuum of behavioral health services and support. The services provided by View Point Health contribute to the reduction in homelessness, reduction in crime and subsequent incarcerations, higher graduation rates, lower dropout rates, decreased ER visits and decreased inpatient admissions.
View Point Health has partnered with the Georgia Department of Behavioral and Developmental Disabilities (DBHDD) to focus on assisting youth in Region 3, ages 6-21 years of age, who are experiencing health crisis and barriers to accessing appropriate care and support, throughout a continuum of care.
This partnership allows us to implement the required and allowable activities by studying, developing, and piloting a formalized multi-agency care coordination protocol focused on removing barriers and expediting access to the right care at the right time for youth and families.
This position offers support, education, advocacy, case management and other assistance to at-risk individuals to identify and remove barriers to obtaining community-based treatment. This position will also provide consultation and expertise to ensure the course of care is coordinated, scheduled, and communicated through the program process to confirm all program participants are supported appropriately.
