Community Support Individual (CSI) - Clayton Center/CYAF - Jonesboro, GA.

Clayton Center proudly provides outpatient treatment to children, adolescents, and adults.  We promote overall health and quality of life for individuals, youth and families through evidenced-based therapies and medication management to reduce the impact of mental illness, behavior challenges, and substance use.

Community Support Individual (CSI) is a service provided to youth who have behavioral health related diagnoses to promote stability and build towards age-appropriate functioning in their daily environments (home, school, social).  The CSI worker meets with youth, family, and school staff to identify needs, provide linkage and community support.

Duties & Responsibilities:

• Providing support to youth and families. 

• Schedule, facilitate, and meet with youths at home or school to provide skill building.

• Meet with families at home or community to link them to community resources (job, Medicaid, food stamps and others).

• Assist youth & family in the development and coordination of the Individual Resiliency Plan (IRP).

• Work with therapists to plan and facilitate meetings and events to provide mental health education and support.

• Assist therapeutic group activities during school breaks.

•  Participate in various meetings with team and school.

• Mental health presentations at school and community.

• Class observation.

• Maintain and submit accurate mileage log(s) and sign in sheet(s).

• Enter all required documentation within 24 hours

• Other related duties as assigned

Minimum Qualifications:

High school diploma/GED and one (1) year of job-related experience providing social or direct care services to groups of special needs individuals in a human service setting or one (1) year of experience at the lower-level Client Service Worker 1 (SSS010) or position equivalent.

Requirements/Competencies:

• Must be able to lift 20 pounds.
• Requires long periods of sitting/standing.
• Must have valid Georgia driver’s license and Motor Vehicle Record in accordance with company policy.
• Candidates for selection must pass a criminal background check (including fingerprinting)
• Must pass a pre-employment drug screen and subject to random drug screens.
