Van Driver & Social Services Asst
Responsibilities
Pathways Center offers Peer and Psychosocial Rehabilitation (PSR) services based out of outpatient clinics around the company. Psychosocial Rehabilitation services include the development of environmental and recovery supports considered essential in improving a person's functioning, learning skills to promote the person's self-access to necessary services, and creating environments that promote recovery and support the emotional and function improvement of the individuals served.
The program operates Monday through Friday, typically 8:30am to 2:30pm, however the transportation duties are before & after these hours, making this a full-time, 8 hour per day role.
Responsibilities for the Van Drive and Social Service Assistant include but are not limited to the following:
□ Supports individuals in building relationships and social networks
□ Provides transportation for client(s) as needed for individual and/or group community outings as well as to and from Peer/PSR-G scheduled group sessions as needed
□ Assists in counseling and advising clients on behavioral problems, daily decision-making, and the resolution of minor problems
□ Participates in Individual Service Plan meetings/Case Reviews consulting with the team to evaluate the client’s needs and plan for continuing services
□ Performs custodial duties in the Peer/PSR-G group areas such as cleaning to ensure safe, sanitized, and clean environment for clients and employees
□ Documents client progress and services provided on a daily basis, reporting any changes in client condition to all members of the client’s direct care treatment team
□ Observes and monitors client's behavior and documents observations and incidents
□ Acts immediately on any crisis, following protocol and basic CPR/first aid training
High School Diploma or GED and one year experience working with clients in a behavioral healthcare organization, and a valid driver’s license OR Associate degree in a related field and a valid driver’s license
