Social Worker, NonLicensed 1
· Provides social services and assistance to improve the physical, social and psychological functioning of patients and their families.
· Identifies psychosocial status, economic and physical needs of patients; assesses and reassesses patient’s support systems, available community resources and other factors to plan, develop, and implement an appropriate plan of care.
· Provides psychosocial assessments, intervention, and care plan to patients of the clinic and assists them and their families in coping with their illness, diagnosis, and/or life situation and meet the continuing health care needs.
· Identifies emotional or behavioral issues and diagnose disorders using information obtained from screening tools, interviews, records and reference materials.
· Counsel individuals and groups (if applicable) in order to modify behavior to improve personal, social and vocational adjustment.
· Develop and implement individual treatment plans specifying type, frequency and duration of therapy.
· Operates as an advocate with all community populations utilizing skills that respect issues of cultural and ethnic diversity and equity for every patient and family.
· Coordinates services to the clinic, community agencies and other professionals; maintains working knowledge of and liaison with community agencies; participates in area activities related to health and social services.
· Collaborates with the health care team and involves the patient/family in the development and implementation of plans.
· Facilitates patient care across a multidisciplinary care continuum which includes interacting with Clinic Providers, and other health care providers to facilitate movement of the patient through the system and arrangement of an appropriate plan of care.
· Utilizing the Avatar system, maintains records and statistics in accordance with department policies.
