Purpose of the Position: Under the supervision of the Maternal Child Home Visiting Manager, assesses pregnant and postpartum (up to 12 months) women, infants and children, identifies their health care needs, and develops and implements plans of care to optimize healthy outcomes. Provides one on one education using the Partners for a Healthy Baby, Baby Basics and Bright Futures Curriculums. Works collaboratively with Social Workers (Non-licensed) Community Health Workers to provide monthly case management to enrolled Maternal Child Home Visiting and Perinatal Health Partnership clients. Documents all client encounters and maintains accurate medical records. Provides care coordination services and education to clients and their families with links to community resources and services. Prepares clients and their families for transition to Children 1st services after 12 months. This position works to promote and protect the health and safety of the residents of Cobb and Douglas Counties.
*This is a grant funded position. While CDPH will seek funding to continue this program, funding for this position will expire on 12/31/2026.
Essential Functions (include but are not limited to):
· Receives referrals and makes initial contact with family. Explains purpose and scope of Maternal Child Home Visiting and Perinatal Health Partnership programs. Schedules intake appointment and meets with client/family face to face during home visits to gather and assess medical and social information, obtaining necessary signatures and authorizations.
· Consults and coordinates care with internal and external health care professionals to develop and evaluate the individualized and comprehensive plan of care.
· Coordinates with care coordinators to contact family for monthly case management by phone or home visit. Provides care coordination nursing services, medical, behavioral health and community resources appropriate for identified needs. Modifies client treatment plans as indicated by client's outcomes, responses, and changing needs.
RN 2 Minimum Qualifications: Bachelor's degree in Nursing from an accredited college or university AND current Georgia license to practice as a Registered Professional Nurse AND CPR certification OR Associate’s degree in nursing from an accredited college or university AND One year nursing experience AND completion of a PHN course within one year of hire AND current Georgia Registered Professional license and CPR certification.
Annual Salary: $61,706.94 / Pay Grade L

Preferred Qualifications:
3 years’ experience with case management/clinical experience in maternal child health nursing.
Work Conditions & Physical Demands:
Works with clients in the home/community 90% of the time. Will travel from the office to client’s homes. Some work completed in the office daily. Must be able to carry educational supplies, laptop or tablet and a 10lb. infant scale to home visits. Overall must be able to carry a minimum of 15 pounds, navigate stairs and provide own transportation (with a valid Georgia driver’s license and auto insurance coverage).
