Assertive Community Treatment (ACT)

 

Provides nursing services in the community for individuals with severe and persistent mental illness as part of multidisciplinary treatment team. Educates individuals and their families, provides service coordination and utilizes crisis prevention and management training as necessary. Services are primarily provided in the community such as homes, places of employment, shelters, jails, or other unstructured environments. Completes documentation of services rendered daily.


NOTE: McIntosh Trail Community Service Board conducts background checks on all final candidates. Hiring of applicants is contingent upon satisfactory results of:

Fingerprint Based Background Check of the Georgia and Federal Criminal History Record 
Database
Drug Screen
Motor Vehicle Report (MVR) for seven years

TB testing will be required upon appointment. If you have previously tested positive for TB, you will be required to bring a copy of your chest x-ray on your first day of orientation.

0-5 years                     10 hours/month (5 hours each pay period)

6-10 years                   12 hours/month (6 hours each pay period)

11 years                      14 hours/month (7 hours each pay period)

Bachelor's degree in nursing from an accredited college or university OR Associate's degree in nursing from an accredited college or university AND current Georgia license as a Registered Professional Nurse AND any related professional work experience.
