Responsibilities
Community Living Support (CLS) Services are individualized supports intended to assist individuals diagnosed with intellectual and/or developmental disabilities with the achievement, preservation, or improvement in skills. The services are offered to individuals who live in their own or family home. Personal care/assistance may be a component part of CLS services. The amount of personal care/assistance is specific to the needs of the individual, as determined by the Supports Intensity Scale, the Health Risk Screening Tool, and other person-centered assessments. The service needs may be addressed in one or more of the following areas: activities of daily living, personal grooming and health care, interpersonal relationships, mobility, home management, and use of leisure time.
Community Residential Services (
As a provider of these services, Pathways Center ensures quality of care through several processes, one of which is through required documentation of each individual's daily activities. Capturing the daily health activity of the individuals served is vital to measuring progress towards the treatment plan and goals outlined in the Individual Service Plans (
The person in this role will provide community residential services to individuals with developmental disabilities. These services are a range of interventions with a particular focus on training and support in one daily living skills to include eating and drinking, toileting, personal grooming and health care, dressing, communication, interpersonal relationships, mobility, home management, and use of leisure time. Services are individually planned and tailored to meet the specific needs of each client.
Responsibilities for the Certified Nursing Assistant (
□ Assists with the development of an individual service/treatment plan for each assigned client
□ Observes and monitors client's behavior and charts observations and incidents
□ Provides training of basic personal and social skills, simple academics, or work skills to clients
□ Arranges for medical care and provides support to ensure that medical care is obtained
□ Ensures and encourages client group activity
□ Provides transportation for client(s) as needed for individual and/or group community outings
□ Limited case management duties
□ Counsels and advises clients on behavioral problems, daily decision-making, and the resolution of minor problems
□ Participates in Individual Service Plan meetings/Case Reviews consulting with the team to evaluate the client’s needs and plan for continuing services
□ Performs housekeeping duties such as cleaning, washing clothes and dishes, and running errands
□ Plans, shops for, and prepares nutritious meals taking into consideration any dietary needs/requirements
Documents client progress and services provided on a daily basis, reporting any changes in client condition to all members of the client’s direct care treatment team
Minimum Qualifications
