The Assertive Community Treatment (ACT) Team is seeking a dedicated and compassionate LPN to provide direct, community-based nursing care and support to adults with severe and persistent mental illness. This role involves working within a multidisciplinary team to help clients manage their health, maintain stability, and achieve independence in their homes and communities

 

Assertive Community Treatment (ACT) uses a multidisciplinary team approach to provide intensive mental health treatment services to individuals with severe and persistent mental illness in their homes and other community settings-24 hours a day, seven days a week, 365 days a year.  ACT is often referred to as a “hospital without walls” and its goal is to keep individuals out of state hospitals, jails, emergency rooms and other institutions.  ACT clients must meet a strict criterion (i.e., diagnosed with a severe and persistent mental illness, unable to maintain activities of daily living, and be unsuccessful in other forms of treatment).  ACT clients often have no resources and no family support and require an intensive and expeditious form of treatment.

 

Under broad supervision, provides general direct nursing care to injured, ill, convalescent, or disabled individuals. Care includes providing medical treatments, administering medication, and giving injections.

 

Duties & Responsibilities:

  • Complete Nursing assessments to determine all healthcare needs. 
  • Educate individuals on medication management and side effects
  • Administer medications as ordered. 
  • Link w/ medical care resources to address Whole Health and Wellness. 
  • Provide feedback on treatment plan development and progress.  Complete documentation to manage recovery goals. 
  • Support on-call procedures based on team schedule. 
  • Complete community-based visits to provide supportive services to the clients and family support.
  • Be responsible for contacting the doctor when any significant change in client medication is indicated.
  • Responsible for completing all forms and paperwork required by the agencies.
  • Participate in training activities to increase skills and become more knowledgeable in methods and techniques used in the care of assigned clients/individuals.
  • Provides on-call, after-hours services on an as needed basis.
  • Spends approximately 65% or more of total scheduled work hours in the community.
  • Other related duties as assigned.

 

 

Minimum Qualifications:

High School diploma/GED and completion of an LPN Program through an approved Georgia Nursing Board; or Vocational/Technical degree from an accredited college and current Georgia license to practice as a Licensed Practical Nurse and six months of nursing experience; or one (1) year of experience required at the lower-level Licensed Practical Nurse 1 (HCT030) or position equivalent

 

Preferred Qualifications: 

  • Two (2) years of nursing experience.
  • One (1) year of experience in a crisis or psychiatric unit.
  • Experience working with psychiatric adults.

 

 

Requirements/Competencies:

  • Must be able to lift 20 pounds.
  • Requires long periods of sitting/standing.
  • Must have valid Georgia driver’s license and Motor Vehicle Record in accordance with company policy.
  • Candidates for selection must pass a criminal background check (including fingerprinting).
  • Must pass a pre-employment drug screen and subject to random drug screens.

 

Note: Some positions may require a valid driver's license.
