The Georgia Department of Behavioral Health and Developmental Disabilities (DBHDD) is the state agency that focuses solely on policies, programs, and services for people with mental health challenges, substance use disorders, intellectual and developmental disabilities, or any combination of these. As Georgia’s public safety net, DBHDD operates six field offices and five hospitals throughout the state within three program areas: behavioral health, developmental disabilities and hospital services.

 

The main office for DBHDD is located in downtown Atlanta.

Job Description

Job Title:  Intake Coordinator - Thomasville

Pay Grade:  K

Job Summary:

This is not a Hybrid position. The selected candidate will be required to report in office full time.

Under general to limited supervision, performs social services function at an advanced level to people applying for Intellectual/Developmental Disability Services, their families and the agencies that support them.  This position reports daily to the assigned office location unless advised otherwise by management.

The Regional Intake Coordinator for Intellectual and Developmental Disability (I/DD) services review all new applications for individuals seeking DBHDD Medicaid waiver and state funded IDD services, assist families with locating required documentation for eligibility determinations, and work closely with Division of Developmental Disabilities Psychologists as they assess individuals and make eligibility determinations.

Job Responsibilities include:
▪ Ensure completion of all aspects of the intake process including, but not limited to, review of applications and associated documents
▪ Making assignments to clinicians
▪ Follow up to obtain missing or incomplete information, eligibility letters, referrals for those who are deemed ineligible, tracking of completed screenings, update and assign to Planning List administration in IDD-C, update and maintain IDD-C tracking for all applications
▪ Respond to families in need of information
▪ Meet with schools and other groups to provide information about the application process
▪ Ability to complete screenings

▪ Clinical review of applications for services and supporting documents
▪ Communication with and education of families and individuals seeking services on the eligibility process and the evidence necessary for determining eligibility
▪ Act as liaison with local school systems and other stakeholders to share and explain available resources and how to access those resources
▪ Assists families and individuals with obtaining required documents from schools or other agencies
▪ May develop, arrange, and present training requested by community partners to create broader understanding of IDD services, waivers, and the process by which one obtains services
▪ Determination of the adequacy of submitted documentation for submission to psychologist
▪ Respond to emergent or unusual circumstances to assist families and individuals in locating supports inside or outside of the DBHDD service system
▪ May be asked to assist with the duties of Regional Social Workers when needed - Other duties as assigned

Preferred Qualifications:

Licensure as a Licensed Clinical Social Worker, Licensed Professional Counselor or Licensed Marriage and Family Therapist and two years of post licensure experience providing counseling, case management, assessments for court depositions, legal proceedings and/or eligibility determinations.

Experience working with people with intellectual disabilities, knowledge of Medicaid Waiver and State Funded Services, as well as extensive use of electronic communication, use of state-wide databased systems and knowledge of social service agencies and local community resources is beneficial.
Experience in the field of Intellectual/Developmental Disabilities
Commitment to internal and external customer service and ability to work as a part of a team
Two years of experience providing case management and coordination services to individuals with I/DD
Experience in human services settings requiring use of clinical assessments, interviewing, and/or written reports of assessment findings
Excellent verbal and written communication skills
Strong skills with Microsoft Word, Excel, and Outlook.

Master's degree in a related area from an accredited college or university AND licensure in the area of assignment AND Two years of related post-license experience OR Master's degree in a related area from an accredited college or university AND licensure in the area of assignment AND Two years of experience at the lower level Social Worker, Licensed 2 (HCP051) or position equivalent.
