Community Residential Services (
As a provider of these services,
The person in this role will provide community residential services to individuals with developmental disabilities. These services are a range of interventions with a particular focus on training and support in one daily living skills to include eating and drinking, toileting, personal grooming and health care, dressing, communication, interpersonal relationships, mobility, home management, and use of leisure time. Services are individually planned and tailored to meet the specific needs of each client.
Responsibilities for the Certified Nursing Assistant (
□ Assists with the development of an individual service/treatment plan for each assigned client
□ Observes and monitors client's behavior and charts observations and incidents
□ Provides training of basic personal and social skills, simple academics, or work skills to clients
□ Arranges for medical care and provides support to ensure that medical care is obtained
□ Ensures and encourages client group activity
□ Provides transportation for client(s) as needed for individual and/or group community outings
□ Limited case management duties
□ Counsels and advises clients on behavioral problems, daily decision-making, and the resolution of minor problems
□ Participates in Individual Service Plan meetings/Case Reviews consulting with the team to evaluate the client’s needs and plan for continuing services
□ Performs housekeeping duties such as cleaning, washing clothes and dishes, and running errands
□ Plans, shops for, and prepares nutritious meals taking into consideration any dietary needs/requirements
Documents client progress and services provided on a daily basis, reporting any changes in client condition to all members of the client’s direct care treatment team
Minimum Qualifications
