Case Manager-Outpatient Services
Responsibilities:
Responsibilities The job responsibilities for the Case Manager position include but are not limited to the following: · Assess the clients’ needs to determine eligibility for the applicable community support service within established time frame · Meets face-to-face with clients to provide community based services including, teaching, training and skill building interventions in a variety of out-of-clinic settings · Completes and submits documentation within required time frames · Attends and provides appropriate input in scheduled treatment team meetings · Participates in developing and reviewing treatment plans · Ensures that services provided are authorized · Accurately documents/charts client progress throughout course of treatment · Coordinates and/or makes appropriate referrals for support services · Schedules follow-up appointments in a timely manner to ensure continuity of services · Recognizes the need for client crisis intervention and reacts appropriately to escalated situations · Develops and effectively maintains a network of appropriate community contacts · Thoroughly monitors available community resources and develops new sources to meet client needs Responsibilities The

Minimum Qualifications:
* High School diploma or GED and three years of experience in a social services related position OR Bachelor's degree in a social services related field and one year experience in a social services related position
* Must have basic working knowledge of symptoms and signs of mental illness
* Must have organizational skills with the ability to multi-task
* A valid driver's license with a clean driving record
