Case Manager - Crisis Stabilization
Responsibilities
Pathways Center Care Campus has two 24/7 Crisis Stabilization Units: one for adults and one for individuals ages 5-17. Pathways Center's Crisis Stabilization Units (
In addition to the crisis stabilization programs, Pathways Center Care Campus has a Crisis Service Center (CSC). The CSC provides on-site and telephonic crisis intervention and 23-hour observation services 24 hours a day, 7 days per week. The CSC operates on a walk-in basis and provides psychiatric/substance abuse related crisis evaluation and brief intervention services to support an individual who is experiencing an abrupt and substantial change in behavior noted by severe impairment of functioning typically associated with a precipitating situation or a marked increase in personal distress. Interventions are provided by licensed behavioral health professionals designed to prevent out of community treatment or hospitalization.
Responsibilities for the Case Manager in this campus include but are not limited to the following:
· Engaging residents and their families/support systems in a welcoming, hopeful and empathetic manner
· Educating each resident and family on the services provided; processes to be followed; and providing information on visitation, program rules, discharge planning and overall orientation to the program
· Gathering information needed to obtain authorization for services
· When working in the Child and Adolescent CSU: is a Liaison with home schools to facilitate continuity in the consumer’s education, facilitates family and natural supports involvement in the child’s care, makes daily follow-up calls to consumer’s family/natural supports to increase engagement, completes PRTF applications, obtains service authorization for services
· Participating in daily staffing meetings with treatment team members
· Assisting with the coordination of aftercare services for individuals in the program
· Assisting residents with accessing community resources to support their recovery process
· Coordinating access to community-based activities for clients on transitional bed status
· Developing discharge and safety plans for each resident
· Documenting case management activities, team staffing notes, and resident's progress into the electronic health record system on a daily basis
· Completing data entry, as needed, for compliance reporting purposes
· Maintains Daily Census report
The individual in this role must be able to exercise judgment and client engagement in coordinating aftercare services; must be able to build and maintain relationships with clients, natural supports, and external service providers; provide timely, detailed documentation of contacts, plans, and progress in individual’s electronic records; must possess positive interpersonal skills with internal and external stakeholders; maintain strong working knowledge of available resources and providers in various regions of the State of Georgia; and be willing to work with all populations served.
Minimum Qualifications
Bachelor's degree in a social services related field and one year experience in a social services related position
