Responsibilities

Pathways Center’s outpatient treatment program provides services that include, but are not limited to, individual, group, and family education and counseling.  The clinic offers comprehensive, coordinated, and defined services that vary in level of intensity.  Outpatient services address a variety of needs such as situational stressors, family relations, interpersonal relationships, mental health issues, life span issues, psychiatric illnesses, addictions, and the needs of victims of abuse and/or trauma.  Outpatient Treatment programs are available to all eligible individuals in our community that meet core customer criteria.

The job responsibilities for the Case Manager position include but are not limited to the following:

·         Assess the clients’ needs to determine eligibility for the applicable community support service within established time frame

·         Meets face-to-face with clients to provide community based services including, teaching, training and skill building interventions in a variety of out-of-clinic settings

·         Completes and submits documentation within required time frames

·         Attends and provides appropriate input in scheduled treatment team meetings

·         Participates in developing and reviewing treatment plans

·         Ensures that services provided are authorized

·         Accurately documents/charts client progress throughout course of treatment

·         Coordinates and/or makes appropriate referrals for support services

·         Schedules follow-up appointments in a timely manner to ensure continuity of services

·         Recognizes the need for client crisis intervention and reacts appropriately to escalated situations

·         Develops and effectively maintains a network of appropriate community contacts

·         Develops monthly tracking system to ensure compliance with service guidelines

·         Thoroughly monitors available community resources and develops new sources to meet client needs

Minimum Qualifications

        A Bachelor's Degree in a human services related field 

         Must have basic working knowledge of symptoms and signs of mental illness

         Must have organizational skills with the ability to multi-task

         A valid driver’s license with a clean driving record

High school diploma/GED and three (3) years of job-related experience in a social services-related position; or one (1) year of experience at the lower level Social Svcs Tech 2 (SST011) or equivalent position.
