General Nature of Work
· Provides outstanding customer service
· Maintains HIPAA standards related to Privacy and Security
· Provides a broad range of social services to individuals with HIV/AIDS and their families.
· Assesses to determine eligibility for the Ryan White Program and enrolls and recertifies as appropriate. Develops plans and assesses effectiveness.
· Links to community resources for assistance. Participates in community consortium meetings.
· Collects and maintains data and completes and submits required reporting
· May work nights and weekends, participate in health fairs, serve on committees, make presentations, or respond to disasters.
· Responds to Public Health Emergencies
Minimum Qualifications
Bachelor's degree in a related field from an accredited college or university, and Two years of experience in social service delivery. (Please mail or bring a copy of College Transcript or Diploma to Interview—must have official transcript prior to start date). Must be able to lift up to 45 lbs, stoop, and bend.)
Preferred Qualifications
· Bachelor's or Master's degree in Social Work
· Experience working with individuals with HIV/AIDS
· ADAP experience
· Case management experience
· More than two years of experience in social service delivery
· Experience using CareWare

This position may require driving company vehicles (Valid driver's license required) and working some evenings and weekends during special events such as health fairs and responding to disasters. Drug screen, criminal background check and immunization records may be required. If selected, male applicants between 18 and 26 years of age must present proof of Selective Service Registration.
