DUTIES AND RESPONSIBILITIES

  • Support individuals in personalized activities of daily living and social skills development.  This may include assisting and supporting individuals in all environments (the community, at work and at home) so that the individual is able to learn, practice and master skills that enable him/her to enhance his/her status by assuming valued social roles.
  • Ensure that the health, safety and well-being of individuals are met.   This will include following all instructions given by the Primary Care Physician, Nursing staff, etc. and may include following a well-balanced diet prepared to meet individual needs.
  • Report changes in the condition of individual and/or any unusual occurrences related to an individual to Supervisor or Management staff in a timely manner.  Report any SIGNIFICANT changes in the condition of an individual IMMEDIATELY.
  • Report environmental risks/hazardous/barriers to Supervisor or Management in a timely manner.
  • Ensure individual’s possessions are safeguarded.  This will include ensuring that the individual’s money and possessions are maintained in a secure manner.
  • Assist individuals in maintaining order and cleanliness in their environment(s).
  • Provide input into the planning and implementation strategies of individual’s plan of care (to include the individual’s Support Plan and Behavior Support Plan).
  • Implement Individual Support Plans (ISP)/Behavior Support Plans (BSP) and strategies as written and at the frequency stated in the ISP/BSP. 
  • Demonstrate accurate data collection procedures and consistent use of described support strategies that promote success.
  • Document services provided including: individualized service goals (from ISP), behavior support tracking, required notes and all other required documentation.
  • Support individuals to manage behavioral challenges and to resolve minor problems.   
  • Complete incident reports using the online system. Ensure the report includes required information (who was involved, what happened, where did it happen, when did it happen, etc.)
  • Transport individuals to medical appointments, community activities or other assigned and desired events while ensuring the safe transport of each individual, adhering to all traffic laws and driving defensively. 
  • Maintain timely and effective communication regarding individuals needs and their environment with appropriate personnel/colleagues.
  • Serve as a role model and mentor, demonstrating socially appropriate behaviors and coach individuals using approved techniques and strategies; use approved intervention techniques when necessary to protect individuals supported and others.
  • Support each individual’s existing relationships and increase their networks of friends and acquaintances.
  • Ensure that all individuals are supported to explore and express their interest for community membership
  • Observe and maintain (HIPAA) confidentiality requirements concerning the affairs of the individuals served, staff and the agency. 
  • Complete all scheduled and required training by the announced deadline, attend and actively participate in home staff meetings.
  • Actively participate in the agency’s ongoing effort to continually improve the quality of services provided and the effectiveness of the agency’s operations by sharing ideas and insights.
  • Facilitate teamwork between all team members via open communication, sharing of responsibilities and a by demonstrating a positive, constructive attitude.
  • Interact professionally and effectively with colleagues, families, guardians, support coordinators and approved internal/external monitors.
  • Complete other duties as assigned


IN RESIDENTIAL SETTINGS ONLY:

  • Provide assistance and/or support with activities of daily living to include, but not limited to: home living activities such as cooking, cleaning, laundry, dishes, household maintenance, community involvement, personal hygiene, leisure activities, communication, personal safety, health maintenance, self-administration of medication administration/medication management and social/behavioral development.
  • Document services provided including: individualized service goals (from ISP), behavior support tracking, required notes, communication logs, medication administration records (MAR) and all other required documentation.
  • Maintain proper communication regarding individuals needs and home environment with appropriate personnel/home colleagues. Read the communication log when coming on shift and filling it out at the end of the shift with pertinent information regarding the individuals and the home.
  • Plan, prepare and grocery shop for meals including following special diets and nutritional instruction. 
  • Follow Supervised Self Administration of Medication (SSAM) and Proxy Caregiver procedures without errors; including, reading and following medical and nursing instructions. 



REQUIRED:

  • High school diploma or GED and any combination of training and experience which would have enabled the applicant to acquire the necessary knowledge, skills and abilities
  • Possess Georgia Driver’s License and acceptable Motor Vehicle Report (MVR)
  • Must have basic computer skills, the ability to serve as an advocate for individuals, understand/interpret written materials, prepare incident reports and strong conflict resolution skills.

Physical Requirements:

  • The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job
  • The job requires regular lifting and carrying up to 30 pounds unassisted, including assisting individuals when required
  • The job requires frequent bending, reaching, repetitive hand movements, standing, walking, squatting and sitting, with some heavy lifting, pushing and pulling exerted regularly throughout a regular work shift.

PREFERRED: 

  • Six (6) months experience working with the intellectually and developmentally disabled population.

 SALARY RANGE:

  • $16.36 Hourly
