Job Description| License Practical Nurse

 

Position Title: License Practical Nurse

Supervisor: Clinical Director

FLSA Status: Exempt

Employment Status: Full -time

Salary Range: Market Rate

We are currently seeking Licensed Practical Nurses to join our team in making a difference in the lives of our patients every day.  We are located in the heart of Middle Georgia State where we enjoy abundant recreational opportunities, and a family friendly environment with good schools and opportunities for the entire family to be involved.

LPN's joining our team must have strong communication skills to work with patients and staff as part of our multi-disciplinary team. Good organizational skills are required. This position is responsible for medication administration, patient care, positive role modeling, documentation of flows, crisis intervention and patient safety.

Duties Summary

Work in a resident homes or clinics which provide care and treatment to patients who are mentally ill. Perform practical nursing care and related technical work in the care, treatment and rehabilitation of mentally ill individuals. Full Performance employees are provided general instructions, and specific instructions or immediate supervision in cases requiring advanced and/or specialized treatment and procedures; perform work independently, receiving specific guidance only in new, unusual or emergency situations; and receive periodic review of work to ensure that acceptable practices are followed; and perform other related duties as assigned.

Responsibilities:

·         Administer nursing care to behavioral health patients

·         Monitor and report changes in patient symptoms or behavior

·         Communicate with collaborating physicians or specialists regarding patient care

·         Educate patients about health maintenance and behavioral health diseases

·         Facilitate referrals to other healthcare professionals and medical facilities

·         Maintain accurate documentation and patient medical records

·         Provide advice and emotional support to patients and their family members

·         Utilizes knowledge of growth and development, psychopathology and psychosocial systems as evidenced by accurate assessments, participation in staffing and case conferences and recognizing and responding appropriately to symptoms and behavior.

·         Manages the medication room, med orders and coordinates with the pharmacist regarding   inventory, prescriptions, etc.…

·         Coordinates medical services as needed and required.

·         Ensures that each resident receives the required care regarding physical health and dental services within the appropriate time frames.

·         Provides leadership for other direct care staff and directs staff to assigned areas based on needs of the residents and program on daily basis.

·         Other duties as assigned

Job Type: Full-time

Licensed Practical Nurse (LPN) Requirements: 

  • Current Georgia LPN license
  • Previous experience in mental health field preferred.
  • At least 21 years of age
  • Possess a valid driver’s license
  • Submit to and pass a pre-employment drug screen and background checks

·         Relevant work experience in Behavioral health patient care is highly preferred.

·         Outstanding interpersonal skills and the ability to work with patients of all ages

Required licenses or certifications: LPN

  • Licensed Practical Nurse

Pre-employment Requirements:

  • Previous experience in mental/behavioral health field.
  • Possess a valid driver’s license
  • Submit to and pass a pre-employment:
    • Drug screen
    • DBHDD Background check
    • Local Criminal Background Check
    • No more than four (4) points on a seven (7) year Motor Vehicle ReporT
      *If more than (4) points an approved DDS course to reduce points must be taken.

High School diploma/GED and completion of a LPN Program through an approved Georgia Nursing Board; or a Vocational/Technical degree from an accredited college and current Georgia license to practice as a Licensed Practical Nurse.
